Instructions to Authors

The Journal requires that authors provide a copy of the manuscript on disk. The hard copy must match the disk. Manuscripts may also be submitted electronically; authors are encouraged to visit the online submission site at http://pros-wiley.manuscriptcentral.com//.
All copy must be double-spaced and have generous margins. Begin each part at the top of a new page. Number all pages in sequence beginning with the title page. Submit the original and three copies of all elements arranged in the following order:
Title page. This should contain the complete title of the paper: the names, titles, and affiliations of all authors; the institution at which the work was performed (specific to the department level); the name, address, telephone and fax numbers, and E-mail address for all correspondence; and a shortened title, not more than 40 characters, to be used as a running head. Each manuscript should have no more than nine authors. If a manuscript was worked on by a group, one or more authors may be named from this group. The other members of the group are not considered authors, but may be listed in an acknowledgment. Contract grant acknowledgment, giving the name of the contract grant sponsor and the contract grant number must be supplied, if applicable, as the last item of the title page. Other acknowledgments must be supplied at the end of the manuscript.
Disclosure Statement. All authors must disclose any affiliations that they consider to be relevant and important with any organization that to any author's knowledge has a direct interest, particularly a financial interest, in the subject matter discussed. Such affiliations inlcude, but are not limited to, employment by an industrial concern, ownership of stock, membership on a standing advisory council or committee, a seat on the board of directors, or being publicly associated with a company or its products. Other areas of real or perceived conflict of interest would include receiving honoraria or consulting fees or receiving grants or funds from such corporations or individuals representing such corporations. This requirement will apply to every sort of article submitted to the Journal, including original research, reviews, editorials, letters to the editor, and any others, and should be disclosed at the time of submission. The simplest remedy for conflict of interest is disclosure. In the Journal, disclosure will henceforth be achieved by the inclusion of a short footnote with each published article. This information will be held in confidence while the paper is under review. It will not be shared with peer reviewers, and it will not influence the editorial decision to accept or reject the manuscript. When an article is accepted for publication, the editors will usually discuss with the authors the manner in which such information is to be presented.
Abstract. This should be a factual condensation of the entire work structured into four paragraphs titled, Background, Methods, Results, and Conclusions. The abstract should not exceed 150 words.
Key words. Supply a list of 3 to 6 key words or phrases (not in title) that will adequately index the subject matter of the article.
Text. The text should follow the format: Introduction, Materials and Methods, Results, Discussion, and Conclusions. Use subheadings and paragraph titles whenever possible. Place Acknowledgments as the last element of the text, before the references. Authors whose first language is not English should arrange for their manuscripts to be written in idiomatic English prior to submission. Please use American Style. If photographs of human subjects are used, a copy of the signed consent form must accompany the manuscript. Letters of permission must be submitted with any material that has previously been published.

In the text, cite references as numerals in brackets. The final list should be in numerical order, by the order they appear in the text, and include full article titles, names of all authors (not et al.), and inclusive page numbers. Abbreviate journal names according to Index Medicus style. Note the following examples: Journal articles: Neri R. Pharmacology and pharmacokinetics of flutamide. Urology 1989;34:19-21.
Books: 2. Goldstein SR. Endovaginal ultrasound, 2nd ed. New York: Wiley-Liss; 1991. 1223 p.

Tables. Each table must have a title. Tables should be numbered in order of appearance with Roman numerals and be referred to by number in the text.
Legends. A descriptive legend must accompany each illustration and must define all abbreviations used therein.
Illustrations. Use illustrations sparingly. They should be numbered in one consecutive series using arabic numerals and be keyed into the text. Photographs should be glossy prints showing high contrast. Drawings should be prepared with India ink. Either the original drawings or good quality photographic prints are acceptable. Name of author, figure number, and an arrow indicating orientation should be typed on a gummed label and affixed to the back of each illustration. Four-color illustrations will be considered for publication. However, the author will be required to bear the cost of their publication. The charge for one page of color is $950. Second, and subsequent pages, up to four, will cost $500 each. The color slide should be supplied, in addition to color prints.
Rapid Communications
The Editorial Board will consider Rapid Communications upon completion of review. This will be published at no cost to the author. This report should contain the equivalent of six double-spaced typed pages (Including any figures, tables, or references) with generous margins.
Disk Submission Instructions
Please return your final, revised manuscript on disk as well as hard copy. The hard copy must match the disk.
The Journal requires authors to deliver the final, revised version of their accepted manuscripts (text, tables, and, if possible, illustrations) on disk. Given the near-universal use of computer word-processing for manuscript preparation, we anticipate that providing a disk will be convenient for you, and it carries the added advantages of maintaining the integrity of your keystrokes and expediting typesetting. Please return the disk submission slip below with your manuscript and labeled disk(s).
Guidelines for Electronic Submission
Text Storage medium. 3-1/2" high-density disk in IBM MS-DOS, Windows, or Macintosh format.
Software and format. Microsoft Word is preferred, although manuscripts prepared with any other microcomputer word processor that can save submitted files as RTF (Rich Text Format) are acceptable. Refrain from complex formatting; the Publisher will style your manuscript according to the Journal design specifications. Do not use desktop publishing software such as PageMaker or Quark XPress. If you prepared your manuscript with one of these programs, export the text to a word processing format. Please make sure your word processing program's "fast save" feature is turned off. Please do not deliver files that contain hidden text: for example, do not use your word processor's automated features to create footnotes or reference lists.
File names. Submit the text and tables of each manuscript as a single file. Tables should be placed at the end of the manuscript, after the references. Name each file with your last name (up to eight letters). Text files should be given the three-letter extension that identifies the file format. Macintosh users should maintain the MS-DOS "eight dot three" file-naming convention.
Labels. Label all disks with your name, the file name, and the word processing program and version used.
Illustrations
All print reproduction requires files for full color images to be in a CMYK color space. If possible, ICC or ColorSync profiles of your output device should accompany all digital image submissions.
Storage medium. Submit as separate files on disk(s) separate from text files. If feasible, full color files should be submitted on separate disks from other image files. 3-1/2" high-density disks, Zip disks, and CDs can be submitted. At authors' request, disks will be returned after publication.
Software and format. All illustration files should be in TIFF or EPS (with preview) formats. Do not submit native application formats.
Resolution. Journal quality reproduction will require greyscale and color files at resolutions yielding approximately 300 ppi. Bitmapped line art should be submitted at resolutions yielding 600-1200 ppi. These resolutions refer to the output size of the file; if you anticipate that your images will be enlarged or reduced, resolutions should be adjusted accordingly.
File names. Illustration files should be given the three-letter extension that identifies the file format used (i.e., .tif, .eps). Files not prepared correctly cannot be used.
Labels. Label all disks with your name, the file names, formats, and compression schemes (if any) used. Hard copy output must accompany all files.
