Instructions to Authors

Address for Contributions
Send manuscripts to the Editor-in-Chief, Dr. Rollin M. Gallagher, Comprehensive Pain and Rehabilitation Center, Graduate Hospital 1st Floor, Pepper Pavilion, 1800 Graduate Hospital, Philadelphia, PA 19146, USA.   Tel: 215-893-4100 Fax: 215-893-2673. E-mail: rollin.gallagher@drexel.edu

Articles

  • Clinical investigations: Present results of original clinical research.
  • Case reports: Describe a single case or a small series. They must be educational and draw attention to important or unusual clinical situations, new treatments, or complications. They will usually not fulfill the same scientific criteria as clinical investigations.
  • Reviews: Comprehensive surveys covering a broad area. They consolidate old ideas and may suggest new ones. They must provide a critique of the literature.
  • Special articles: On subjects not easily classified above (e.g., articles on history, education, demography, ethics socioeconomics, etc.).
  • Letters to the editor: These may offer criticism of published material, but must be objective, constructive, and educational. A few references, a small table, or relevant illustrations may be used.
Form of Manuscripts. Submit, in English, double-spaced, an original and three copies typed in letter-quality type on standard-sized white bond paper with a 4-cm (1.5") left margin. Four complete sets of illustrations must also be included. The author should retain a copy of the manuscript for reference, as accepted manuscripts are not returned.

Details of Style. Follow guidelines set by American Medical Association Manual of Style, Eighth Edition, Williams and Wilkins, 1989. Double-space throughout, including title page, abstract, text, acknowledgements, references, legends for illustrations, and tables. Start each of these sections on a new page, numbered consecutively in the upper right-hand corner, beginning with the title page.

The body of the text must be in the following sequence: Introduction, Methods, Results, Discussion, and Conclusions.

On the title page type the full names, highest academic degrees, and specific affiliations of all authors. Indicate the author to whom correspondence should be directed, and the telephone and fax numbers, as well as the e-mail address.

The abstract, on the page following the title page, must be 250 words or less, under the following headings, as appropriate: Objective, Design, Setting, Patients, Interventions, Outcome Measures, Results, and Conclusions (JAMA 1992;267:42-4). Abstracts are necessary for all papers, Up to six key words must be provided with the abstract.

Drug names: Use generic names only in referring to drugs. If the trade name is necessary, e.g., in bio-availability studies, indicate it in parentheses.

Abbreviations: Keep abbreviations to the minimum, and define each at its first use. Do not use abbreviations in the abstract.

References: Number references in the order of their use in the text: do not alphabetize. Identify with Arabic numerals inside parentheses.

Use the Index Medicus reference style (see "Uniform Requirements for Manuscripts Submitted to Biomedical Journals." Ann Intern Med 1988;108:258-65). For abbreviations of journal names, refer to List of Journals Indexed in Index Medicus. Provide article titles and inclusive pages. Accuracy of reference data is the responsibility of the author.

Journal Article: Warfield CA, Biber MP, Crews DA, Dwarakanath GK. Epidural steroid injection as a treatment for cervical radiculitis. Clin J Pain 1988;4:201-4.

Chapter in book: Pilowsky I. Psychodynamic aspects of the pain experience. In: Sternback RA, ed. The psychology of pain. Second Edition. New York: Raven Press, 1986:181-95.

Book: Price DD. Psychological and neural mechanisms of pain. New York: Raven Press, 1988.

Tables: Double-spaced, on separate sheets of standard-sized white bond paper. Title all tables and number them in order of their citation in the text. Any notes should appear at the bottom of the table.

Illustrations. Illustrations must be unmounted, and identified on the back, in soft lead pencil, with author's name, figure number, and top plainly marked. Graphs and drawings should be 5 ¡Á 7 inches (approximately) glossy prints and should be of professional quality. X-rays or clinical photos: Remove all markings from x-rays before photographing (such as patient's initials, dates, clinic numbers). Any arrows or lettering must be applied with a professional product (such as Chartpak). These identifying marks should be made large enough to be seen when the photo is reduced. Sequences of radiographs should be of the same magnification. The subject should be centered in clinical photographs. Crop out extraneous material and background. Each figure should have a separate fully explicit legend; all sections of the figure and all abbreviations and symbols used should be clearly defined. Color illustrations will be charged to the authors.

Note: Photography in which a person's face is recognizable must be accompanied by a letter of release from the subject, explicitly granting permission for publication. For figures previously published elsewhere, written permission must be obtained for reproduction from the author and copyright holder.

Proofs and Reprints. Proofs must be returned within 3 days of receipt; late return may cause a delay in publication of an article. Please check text, tables, legends, and references carefully. To expedite publication, page proofs rather than galleys will be sent to the author, and it may therefore be necessary to charge for alterations other than corrections of printing errors.

An order form for reprints will be included with proofs. Return proofs and reprint order form to the address indicated in instructions with the proofs (mail via first class; air mail for overseas contributions).

Copyright, Authorship Responsibility, and Financial Disclosure. The American Academy of Pain Management will hold copyright on all published articles. In view of present U.S. copyright laws, all authors will be asked to sign Copyright Release, and Authorship Responsibility and Financial Disclosure statements. All forms will be sent to the corresponding author by the office of the Editor-in-Chief when a manuscript is accepted for publication, and must be signed by all authors.

Manuscript Checklist.

  1. Include original manuscript and three photocopies; include four complete sets of illustrations.
  2. Put references in proper format in numerical order, making sure each is cited in the text.
  3. Provide an abstract with appropriate headings.
  4. Include complete consent forms for patient photographs.
  5. Include consent forms for previously published illustrations and tables.
  6. Designate a corresponding author and provide an address, telephone number, and when available, a fax number.
Blackwell Science, Inc. will accept submission of electronic versions of manuscripts in conjunction with the traditional paper version. The preferred storage medium is a 5 1/4- or 3 1/2-inch disk in an MS-DOS compatible format. Files in a Macintosh compatible format will also be accepted.

Files should be submitted in one of the following standard word processing formats: Microsoft Word, WordPerfect, WordStar, or XYWrite. While we can convert from other word processing formats, the vagaries of the conversion process may introduce errors. Do not submit ASCII text files.

Each submitted disk must be clearly labeled with the name of the author, item title, journal title, type of equipment used to generate the disk, word processing program (including version number), and filenames used.

The file submitted on disk must be the final corrected version of the manuscript and must agree with the final accepted version of the submitted paper manuscript. The disk submitted should contain only the final version of the manuscript. Delete all other material from the disk.

Please follow the general instructions on style/arrangement and, in particular, the reference style as given in "Instructions to Contributors."

Note, however, that while the paper version of the manuscript must be presented in the traditional double-spaced format, the electronic version will be typeset and should not contain any extraneous formatting instructions. For example:

  • Use hard carriage returns only at the end of paragraphs and display lines (e.g., titles, subheadings)
  • Do not use an extra hard return between paragraphs
  • Do not use tabs or extra space at the start of a paragraph or for list entries
  • Do not indent runover lines in references
  • Turn off line spacing
  • Turn off hyphenation and justification
  • Do not specify page breaks, page numbers, or headers
  • Do not specify typeface
  • Care should be taken to correctly enter "one" (1) and lower case "el" (l), as well as "zero" (0) and captial "oh" (O)

Please observe the following conventions concerning dashes: Use a single hyphen with space before it for a minus sign, use a double hyphen (with space before and after) to indicate a "long dash" in text, and a triple hyphen (with no extra space) to indicate a range of numbers (e.g., "23---45").

Illustrations and tables will be handled conventionally. However, figure and table legends should be included at the end of the electronic file.

Non-standard characters (Greek letters, mathematical symbols, etc.) should be coded consistently throughout the text. Please make a list of such characters and provide a listing of the codes used
