The Milbank Quarterly
The Milbank Quarterly is devoted to scholarly analysis of significant issues in health and health care policy. It presents original research, policy analysis, and commentary from academics, clinicians, and policy makers. The in-depth, multidisciplinary approach of the journal permits contributors to explore fully the social origins of health in our society and to examine in detail the implications of different health policies.
Topics addressed in The Milbank Quarterly include the impact of social factors on health, prevention, allocation of health care resources, legal and ethical issues in health policy, health and health care administration, and the organization and financing of health care.

Instructions to Authors
The Milbank Quarterly is devoted to scholarly analysis of significant issues in health and health care policy. It presents original research, policy analysis, and commentary from academicians, clinicians, and policy makers. The in-depth, multidisciplinary approach of the journal permits contributors to explore fully the social origins of health in our society and to examine in detail the implications of different health policies. Topics addressed by The Milbank Quarterly include the impact of social factors on health, prevention, allocation of health care resources, legal and ethical issues in health policy, hospital administration, and the organization and financing of health care.
Manuscript Submission Four copies of manuscripts should be sent to Editor, The Milbank Quarterly, Milbank Memorial Fund, 645 Madison Avenue, New York, NY 10022. Alternatively, manuscripts in Word or WordPerfect may be transmitted as e-mail attachments to Kate Frank (quarterly@milbank.org). Manuscripts are accepted for consideration with the understanding that they represent original, unpublished work, that they are not being considered for publication elsewhere, and that they will not be widely distributed while under review. Abstracts published in conjunction with scientific meetings are not considered to be publications. Manuscripts will not be returned to authors. For a more detailed discussion of the Quarterly's policy, see "Prior Publication Policy." Authors will be requested to sign a transfer of copyright to the Milbank Memorial Fund, so that the authors and publishers may be protected from misuse of copyrighted material. Requests from others for Permission to reprint copyrighted material are almost always honored by the Quarterly if the author also gives permission and the Quarterly is given proper credit. Accepted manuscripts are copyedited according to Quarterly style and returned to the author for approval. Authors are responsible for all statements made in their work, including changes made by the copy editor and authorized by the corresponding author.
Specifications Manuscripts, including references, must be typed double-spaced. The length of submitted material should not exceed 30 pages, including references and abstract. On a separate title page include the name, academic degrees, and affiliation of all authors. Also provide the complete address and telephone number of the author responsible for negotiations and the author to whom reprint requests should be sent. List up to four key words for indexing. To facilitate blind review, include a second title page with only the manuscript title. On a separate page include an abstract of no more than 125 words. Tables should be numbered in Arabic numerals. Each figure should have a separate legend. References should be listed at the end of the article alphabetically (chronologically, with the earliest year first, for a particular author) and, unnumbered. The listings must be complete, and unabbreviated, as in the following examples:

Itemize all authors; do not use "et al". In the text, cite the author(s) surname(s) and year of publication within parentheses, e.g., (Blake 1983). For two or three authors write out the surnames, followed by the date, e.g., (Blake, Jones, and Aspinall 1989). When there are more than three surnames, write out the first one followed by "et al." unless there are two or more references with the same initial name and date; in such instances list as many authors as necessary to distinguish the items. The dates for multiple citations by the same author(s) should be separated by commas, e.g., (Retchin and Brown, 1990, 1991). The Quarterly discourages the use of footnotes except when citing legal sources. Authors are required to disclose any commercial or other associations that might pose a conflict of interest or the appearance of conflict of interest in connection with the submitted article. All funding sources, receipt of royalties, or consultantships associated with the work should be acknowledged on the title page, as should institutional or corporate affiliations of the authors.
