Population Health Management

This authoritative peer-reviewed journal, Population Health Management, reflects the expanding scope of health care management and quality.  The Journal delivers a comprehensive, integrated approach to the field of population health and provides information designed to improve the systems and policies that affect health care quality, access, and outcomes, thereby improving the health of an entire population. 

Population Health Management is broadening its focus to include the impact of social, cultural, economic, and environmental factors on health care systems and practices. 

Each issue includes:  Original Research Articles, Clinical Reports & Case Studies, and much more

The Official Journal of DMAA: The Care Continuum Alliance. Members of the society can access the e-journal via the "Members Only" section of the society website.

MANUSCRIPT CRITERIA AND INFORMATION

 

DISEASE MANAGEMENT (DM) is an international, peer-reviewed publication that offers a comprehensive, integrated approach to the most current clinical and business developments transforming healthcare in the field of disease management. DM covers a broad spectrum including: clinical reports and case studies; practical aspects of developing and implementing disease management initiatives; perspectives and insights from all stakeholders; and interviews with leading experts in disease management.  Manuscripts submitted to DM should meet the following criteria: the material is original, the writing is clear, the study methods are appropriate, the data are valid, the conclusions are reasonable and supported by the data, the information is important, and the topic has general disease management interest. We will assess a paper's eligibility for publication based on these basic criteria.

Manuscripts are considered with the understanding that they have not been published previously in print or electronic format, and are not under consideration by another publication or electronic medium, except in the form of an abstract of not more than 400 words. Prior abstract presentations should be described in a footnote to the title. Submissions should be accompanied by a letter requesting evaluation for publication.


Manuscript Submission and Copyright Agreement Form

The Copyright Agreement form (available from web site at http://www.liebertpub.com/media/content/transfer_of_copyright.pdf) should be submitted once your paper has been accepted for publication.  Manuscripts cannot be published without this form. The corresponding author is responsible for obtaining signatures of coauthors. Authors not permitted to release copyright must still return the form signed under the statement of the reason for not releasing the copyright.  Upon acceptance of your paper, please fax the Copyright Agreement form to 914-740-2108.

 

PREPARATION OF MANUSCRIPT

Manuscripts should be submitted in Microsoft Word double-spaced in an 8 ½ x 11 format. Leave ample margins on both sides, top and bottom.

The title page should include the authors' names and affiliations, the source of a work or study (if any), and a running title of approximately 45 characters. We require the full mailing address and contact information (telephone, fax, and email address) for EACH author listed on your article. Please include the address(es) either on the title page of your paper, or on a separate sheet. Please also indicate the corresponding author. The second page should consist of an abstract of not more than 250 words, which should be self-explanatory without reference to the text. Number pages consecutively. At the end of the article, give the name and address of the individual to whom reprint requests should be directed.

 

TABLES AND ILLUSTRATIONS

 

Prepare each table with its title on a separate page. Use Arabic numerals to number tables. Each table must stand alone, i.e., contain all necessary information in the caption, and the table itself must be understood independently of the text. Details of experimental conditions should be included in table footnotes. Information that appears in the text should not be repeated in tables, and tables should not contain data that can be given in the text in one or two sentences.

Please submit artwork according to the following guidelines:

  • Do not include any illustrations as part of your text file.
  • Do not prepare any figures in Word as they are not workable.
  • Line illustrations must be submitted at 1200 DPI.
  • Halftones and color photos should be submitted at 300 DPI.
  • Please submit only TIFF or EPS files.
  • Color art must be saved as CYMK not RGB or INDEX. If RGB files are submitted, the files will be converted to CYMK, and some slight color variation may occur.
  • Do NOT submit PowerPoint or Excel files. Adobe is the software of choice

In order to expedite your submission as quickly and efficiently as possible we ask that all artwork be checked using Digital Expert before submitting.  This is a free tool that will ensure that you prepare and submit quality digital materials suitable for print.

ADDITIONAL INFORMATION ABOUT ART FILES
 
Converting Word or Excel files:  Perhaps the best and easiest way to convert Word or Excel files into a format which is suitable for print is to scan them using the guidelines below:

 

  • All files should be scanned at 100% size.
  • 300 dpi
  • Final color mode: cmyk
  • save file as: .tif or .eps

AUTHOR DISCLOSURE

Institutional affiliations, as indicated on the title page, should include all corporate affiliations and any funding sources that support the work. Other types of affiliation, including consultantships, honoraria, stock ownership, equity interests, arrangements regarding patents, or other vested interests should be disclosed in the Acknowledgments section.


REFERENCES

References must be typed double spaced and numbered consecutively as they are cited in the text (using superscript numbers). References appearing for the first time in tables and figures must be numbered in sequence with those cited in the text where the table or figure is mentioned. Use journal abbreviations as provided by Medline. List all authors when there are six or less. When there are more than six authors, list the first three, followed by et al. If references to personal communications or unpublished data are used, they are not to be in the list of references. They should be referred to in the text in parentheses (C.O. Tucker, personal communication). Among the references, include papers accepted but not yet published; designate the journal, and add "In Press."

PERMISSIONS

The author must obtain permission to reproduce figures, tables, and text from previously published material. Written permission must be obtained from the original copyright holder (generally the publisher, not the author or editor) of the journal or book concerned. An appropriate credit line should be included in the figure legend or table footnote, and full publication information should be included in the reference list. Written permission must be obtained from the author of any unpublished material cited from other laboratories, and should accompany the manuscript.

REPRINTS

Reprints may be ordered by using the special reprint order form that will accompany the proofs. Reprints ordered after the issue is printed will be charged at a substantially higher rate.

LETTERS TO THE EDITOR

Letters discussing a recent DM article are welcome and must be received within 4 weeks of the article's publication. All letters are to be typewritten and not exceed 500 words.
