MANUSCRIPT PREPARATION
Submit four complete sets of manuscript (along with one disk). Type complete manuscript double-spaced, on one side only, on 8-1/2 x 11 inch white paper, with 1-inch margins. Attach full-page photocopies of all figures to the back of each manuscript.
Place first author's last name and page number at the top of each page. Cite references, tables, and illustrations consecutively as they appear in the text.
Arrange manuscript as follows: (1) title page, (2) abstract, (3) body of paper, (4) acknowledgments, (5) references, (6) tables, (7) figure legends, (8) figures, (9) permissions.
Computer Disks: Authors must submit a 3-1/2" HD/DD computer disk to the editorial office. Please observe the following criteria. Send only hard copy when first submitting your paper for acceptance and review. When your paper has been refereed, revised if necessary, and accepted, send a disk containing the final version with the final hard copy. Make certain that the disk and hard copy match exactly. Specify what software was used, including which release, e.g., WordPerfect 6.1. Specify what computer was used (IBM compatible PC, Apple Macintosh, etc.) The article file should include all textual material (text, references, tables, figure captions, etc.) and separate illustration files, if available. The file should follow the general instructions on style/arrangement and, in particular, the reference style of this journal as given in the Instructions to Authors. The file should be single spaced and should use the wrap-around end-of-line feature, i.e., returns at the end of paragraphs only. Place two returns after every element, such as title, headings, paragraphs, and figure and table callouts. Be sure to keep a back-up disk for reference and safety.
Title Page: Provide a complete title (limit: 120 characters) and a short title (limit 45 characters) for use as a running head. List complete names of all authors and name and full location of department and institution where work was performed. List grant support and other assistance. (Acknowledgments are listed at the end of the text.) If paper was presented at a scientific meeting, provide a footnote giving name of meeting, date, and location. Provide name, complete mailing address, telephone number, fax number, and e-mail address of corresponding author.
Abstract: Do not use abbreviations, footnotes, or references. Limit: 200 words. Submit 3 to 5 key words using standard Index Medicus terminology.
Body of Paper: Organize in the following manner: (1) introduction, (2) material and methods, (3) results, (4) discussion, (5) conclusion.
Describe ethical guidelines followed (for human or animal studies); cite approval of institutional human research review committee or animal welfare committee; describe in detail hazardous procedures or chemicals involved, including precautions observed. Outline statistical methods used. Identify drugs and chemicals used by generic name. If trademarks of instruments and other devices are mentioned, list manufacturer and city. Type footnotes on the manuscript page on which they occur.
Acknowledgments: Limit to those persons who have contributed to the scientific development or production of the manuscript.
References: Cite references in order of appearance in text using superior Arabic numbers. Double check that the numbered references at the end of the article correspond. Cite personal communications and unpublished data in text, not in references. Journal abbreviations must conform to those used in Cumulated Index Medicus.

The author is responsible for the accuracy and completeness of the references.
Tables: Type each table on a separate sheet. Number with Arabic numerals according to order of citation. Table number and brief descriptive title appear above the table, explanatory notes below.
Figure Legends: Legends are required for illustrations and should be typed double spaced on a separate page. Number according to order of citation. Provide enough information to permit interpretation of data without reference to text. Illustrations taken from other publications must be acknowledged by a complete credit line in the legend.

Please visit the following website for information on preparing digital artwork: http://www.elsevier.com/homepage/sab/artwork/index.html . Maximum size: 5 x 7 inches. Photographs of identifiable patients must be accompanied by a signed release to publish from the patient.
Protect figures from damage by placing each set in a separate envelope with a piece of cardboard to keep them flat. Do not paper clip figures. Do not mount multiple part figures; however, a suggested layout may be included.
Identify each figure with first author's last name, figure number, and an arrow indicating the top on a self-adhesive label affixed to the back of each figure.
Contributors will be required to pay for the printing of color illustrations.
