Please read all the instructions to authors before submitting.
Manuscript Submission and Copyright Agreement Form
The Copyright Agreement form (available from web site at http://www.liebertpub.com/media/content/transfer_of_copyright.pdf.) should be submitted once your paper has been accepted for publication. Manuscripts cannot be published without this form. The corresponding author is responsible for obtaining signatures of coauthors. Authors not permitted to release copyright must still return the form signed under the statement of the reason for not releasing the copyright. Upon acceptance of your paper, please fax the Copyright Agreement form to 914-740-2108.
Preparation of Manuscript
Include a running title of about 45 characters. An abstract of not more than 250 words, which should be self-explanatory without reference to the text should also be prepared. The abstract should not contain subheadings nor specific citations to the literature. Main subject headings should be centered in all capital letters. Secondary headings should be flush left-hand margin initial cap (first word only) italics. Tertiary headings should be flush left-hand margin initial cap (first word only) bold, followed immediately by text. Professional degrees (M.D., Ph.D., RN, etc.) must be included for all authors on the title page.
Text
Use only standard abbreviations, which can be found in the AMA’s Manual for Authors & Editors. At first usage, spell out terms and give abbreviations in parentheses. Thereafter, use only abbreviations. It is not necessary to spell out standard units of measure, even at first usage. Use generic names for drugs if possible. If you wish to use a proprietary drug name the first time it appears, use the generic name followed by the proprietary name, manufacturer, and city in parentheses.
AUTHOR DISCLOSURE
All authors are expected to disclose any institutional or commercial affiliations that might pose a conflict of interest regarding the publication of a manuscript. Institutional affiliations, as indicated on the title page, should include all corporate affiliations and any funding sources that support the work. Other types of affiliation, including consultantships, honoraria, stock ownership, equity interests, arrangements regarding patents, or other vested interests should be disclosed in the Acknowledgments section.
References
References must be numbered consecutively as they are cited in the text using superscript numbers after the period. References appearing for the first time in tables and figures must be numbered in sequence with those cited in the text where the table or figure is mentioned. Use journal abbreviations as provided by Medline, National Library of Medicine. List all authors when there are six or less. When there are more than six, list the first three, followed by et. al. If references to personal communications or unpublished data are used, they are not to be in the list of references. Instead, they should be referred to in the text in parentheses. (F.P. Hazeltine, personal communication). Include among the references papers accepted but not yet published; designate the journal and add "In Press." Please refer to the AMA’s Manual for Authors & Editors or a previous issue of the Journal for assistance with the preparation of references.
Letters to the Editor
Letters should be written as a continuous narrative, without abstract or subheadings. There is no word limit for Letters, but they are generally limited to three printed pages. There is a two figure and/or table limit, and a maximum of ten references.
Reprint Address
Following the references, provide an "Address reprint requests to:" section that includes the name and complete address of the person to whom reprint requests should be sent. An e-mail address may also be included if desired.
Legends
Legends for illustrations may be entered when individual files are uploaded or as a separate file. In the legend, provide explanations for any abbreviations, arrows, etc. on the art work. If the illustration is taken from a copyrighted publication, permission must be obtained and credit given in the legend.
Tables
Tables should be double-spaced, each as a separate file, and provide a title for each table. Cite tables in sequence in the text. Explain abbreviations used in the body of the table in footnotes. If the table is taken from a copyrighted publication, permission must be obtained and credit given in a footnote.
Illustrations
Cite figures consecutively in the manuscript within parentheses. Example: These keratotic areas can be confused with condyloma (Fig. 8A). The Journal publishes all illustrations in black and white. Requests for color may be submitted; the cost will be incurred by the authors. Please contact Publisher for color cost. Electronic submission of art is acceptable with the following caveats:
• Black and white line illustrations should be submitted at 600 DPI. • Halftones and color should be submitted at a minimum of 300 DPI. • Save art as either TIFF or EPS files. Color art must be saved as CYMK—not RGB. • Do NOT submit PowerPoint or Excel files. Adobe is the software of choice.
Permissions
The author must obtain permission to reproduce figures, tables, and text from previously published material. Written permission must be obtained from the original copyright holder (generally the publisher, not the author or editor) of the journal or book concerned. An appropriate credit line should be included in the figure legend or table footnote, and full publication information should be included in the reference list. Written permission must be obtained from the author of any unpublished material cited from other institutions. Please include copies of all permission letters obtained. All permissions’ listings must be shown in the manuscript; they cannot be entered on proofs.
Reprints
Reprints may be ordered by using the special reprint form that will accompany the proofs. Once page proofs are received by the corresponding author, they should be read carefully and returned promptly to the publisher with the author’s approval indicated by initialing. Reprints ordered after the issue is printed will be charged at a substantially higher rate.
