Papers are accepted on the understanding that they have not been and will not be published elsewhere. It is a condition of publication that authors grant Blackwell Publishing the exclusive licence to publish all articles including abstracts. Papers will not be passed to the publisher for production unless the exclusive licence to publish has been granted. Manuscript style
The typescript should be on A4 paper on one side only, double spaced with a wide margin on either side. Manuscripts should usually not be longer than 5000 words. In the case of short reports these should normally not exceed 2000 words. Manuscripts may be submitted on disk. Further details can be obtained from the publisher.
Author material archive policy. Please note that unless specifically requested, Blackwell Publishing will dispose of all hardcopy or electronic material submitted two months after publication. If you require the return of any material submitted, please inform the editorial office or production editor as soon as possible if you have not yet done so.
Title page This should contain a concise title of the article, names and qualifications of authors, their affiliations and the full postal address, fax and telephone number of an author to whom correspondence can be addressed.
Abstract This should be on a separate page and should not exceed 300 words. Where appropriate authors should cover the following areas: objective, design, setting, respondents/subjects, intervention, or description of the variables under study, main outcome measures if relevant, results and conclusions. The abstract should be followed by up to six key words.
Main text This should begin on a separate page, and include an introduction, methods, results and discussion section. Where possible authors should avoid using abbreviations and footnotes. The use of nondiscriminatory language is encouraged and spellings should conform with that used in the Concise Oxford Dictionary of Current English.
References These should be in the Harvard style. In the text the authors names should be cited followed by the date of publication, e.g.. Smith & Parker (1991). Where there are three or more authors, the first authors name followed by et al. should be written in the text, e.g., Williams et al. (1991). The reference list should be prepared on a separate sheet, listed in alphabetical order. The references should list authors surnames and initials, date of publication, title of article, name of journal or book, volume number or edition, editors, publisher and place of publication.
In the case of an article, page numbers should be included routinely, page numbers from books need only be included when referencing direct quotes or paraphrases. Unpublished work should only be cited in the text only. Only references to articles genuinely in press should be included in the reference list.

Illustrations These should be referred to in the text as figures using Arabic numbers e.g., Fig. 1, Fig. 2, etc., in order of appearance. Each figure should be marked on the back with its appropriate number, together with the name(s) of the author(s) and title of the paper. Photographs should be unmounted glossy prints in black and white, labelling should be clearly indicated on an overlay or photocopy. Line drawings should be on a separate sheet with lettering (no less than 4 mm high for 50% reduction) on an overlay or photocopy, dot matrix illustrations are not permitted. Each figure should have a legend clearly describing it; the legends should be grouped on a separate page at the end of the manuscript. Colour illustrations It is the policy of Health and Social Care in the Community for authors to pay the full cost for the reproduction of their colour artwork.
Therefore, please note that if there is colour artwork in your manuscript when it is accepted for publication, Blackwell Publishing require you to complete and return a colour work agreement form before your paper can be published. Tables These should include only essential data. Each table must be typewritten on a separate sheet and should include a clear title, tables should be numbered in the same way as figures and listed on a separate page at the back of the manuscript.
Acknowledgements These should be brief and must include reference to sources of financial and logistical support. Author(s) should clear the copyright of material they wish to reproduce from other sources and this should be acknowledged.
Units Measurements where appropriate must be in SI units. Units, Symbols and Abbreviations (Royal Society of Medicine 1988) is a useful guide. Disks
The final copy and the file on disk must be the same. A file description form (containing information on the type of computer used etc) should also be completed and sent with the disk: (http://www.blackwellpublishing.com/pdf/fdf.pdf). Do not use the carriage return (enter) at the end of lines within a paragraph. Turn the hyphenation option off. Always enclose a hard copy of figures and tables. Disks will not be returned to authors.
Review papers These will be actively encouraged. Prospective authors should initially discuss their proposals with the editor.
Letters to the Editor Letters should be succinct and must relate to articles which have been published in the journal. They must be signed by all signatories. The Editor reserves the right to shorten letters if necessary. Three copies should be sent to the Editor. Proofs of letters will not be sent to the authors.
Special issues From time to time the Editor may commission on a special issue of the journal which would take the form of a number of papers devoted to a particular theme. Special issue organisers will be expected to produce introductory and concluding discussion sections.
Page proofs These must be returned to the production editor within 3 days of receipt. Only typographical errors and other essential changes can be made at this stage. Major text alterations cannot be accepted.
The corresponding author will receive an email alert containing a link to a web site. A working e-mail address must therefore be provided for the corresponding author. The proof can be downloaded as a PDF (portable document format) file from this site. Acrobat Reader will be required in order to read this file. This will enable the file to be opened, read on screen, and printed out in order for any corrections to be added. Further instructions will be sent with the proof. Proofs will be posted if no e-mail address is available; in your absence, please arrange for a colleague to access your e-mail to retrieve the proofs.
Offprints Authors will be provided with electronic offprints of their paper. Electronic offprints are sent to the first author at his or her first email address on the title page of the paper, unless advised otherwise; therefore please ensure that the name, address and email of the receiving author are clearly indicated on the manuscript title page if he or she is not the first author of the paper. Paper offprints may be purchased using the order form supplied with proofs.
