__Include number of references and number of figures and tables below word counts.
__Full names, degrees, and hospital or university affiliation of EACH author. When authors are affiliated with more than one institution, affiliation should be identified using a, b, c, etc. connecting each author to the appropriate institution. If present affiliation is different from that under which the work was done, both should be given.
__List all sources of support for research.
__At bottom of page, provide address, telephone and fax numbers, and e-mail address of author to whom all communications should be sent.

Abstract and Key Words
__Abstract should be typed, double-space, on separate page (word limitation above). Include subheads of Background, Methods, Results and Conclusion.
__Below abstract, include a list of 3-5 key words for indexing purposes. Use terms from the medical subject headings (MeSH) list in Index Medicus (See www.nlm.nih.gov, Library Services/Collections); if suitable MeSH terms are not available for recently introduced terms, present terms may be used.

Text
__The following subheads should be included in all Original and Special Contributions: Introduction, Methods, Results and Discussion.
__Methods section should include a statement that the experimental protocols and the process for obtaining informed consent (in human studies), were approved by the appropriate institutional review committee.
__For studies on animal subjects, Methods section should identify species, strain, and supplier/source.
__For studies on human subjects, Methods section should include Study Population subheading, under which demographics of study population are defined.
__Use acronyms after they are first spelled out in full (in abstract or text). Avoid jargon.
__Measurements should conform to the "Uniform Requirements for Manuscripts Submitted to Medical Journals" (see "Manuscripts" above.)
__Suppliers of specific instruments or drugs should be noted, including the company name and city. All drugs should be referred to by their generic names.

Acknowledgements

References
__References should be typed double-space.
__Ensure that references are complete and accurate. All authors must be listed. Identify abstracts as such.
__References should conform to the style below for a journal article (1), book (2), and a chapter in a book (3).
1. Lender D, Arauz-Pacheco C, Breen L, Mora-Mora P, Ramirez LC, Raskin P: A double-blind comparison of the effects of amlodipine and enalapril on insulin sensitivity in hypertensive patients. Am J Hypertens 1999;12:298-303.
2. Baron EJ, Chang RS, Howard DH, Miller JN, Turner JA: Medical Microbiology: A Short Course. Wiley-Liss, New York, 1994, pp 39-41.
3. Elias M, Elias P: Motivation and activity, in Birren JE, Schaie KE (eds): Handbook of the Psychology of Aging. New York, Van Nostrand, 1976, pp 357-359.
__Journal names should be abbreviated as listed in Index Medicus. (See www.nlm.nih.gov, Library Services/Services); Personal communications and unpublished data should be included within the text of the manuscript, not as references.

Tables and Figures
__Tables are typed double-space on separate sheets of 8? x 11" paper. Tables should contain a title of no more than 15 words.
__Figures should be camera-ready, unmounted black-and-white prints or laser printouts on white background. Shades of gray should be sufficiently distinguished. Faxes and photocopies are not acceptable. Do not send original artwork.
__Figures will be sized to fit into the typeset article and will be limited to a width of no more than 2 columns. Authors should proportion figures so that they will fit into one column, unless the information content of the figures requires two columns to adequately present the data. Since text may lose legibility with adjustment, a plain font should be selected in the largest size that will fit all necessary information.
__When keys are used in figures, use bold patterns; avoid shading.
__Figures and tables should be numbered consecutively and cited in the text.
__On separate page, legends should be typed double-space for each figure.
__All abbreviations used in an illustration should be expanded in a legend.

Permissions
__For any previously copyrighted figure or table, a letter from the copyright holder giving permission for AJH to reproduce the material must be included prior to publication. Permission must include print and electronic rights. Any related fees are the responsibility of the author.
