Instructions to Authors

 With Our New Digital Manuscript Process

AQCH is pleased to announce Internet access for submission and review of manuscripts. Cadmus Rapid Review is providing AQCH's editorial office with state-of-the-art peer review tracking software and hosting. The Rapid Review software is a Web-based application that allows all participants in the editorial process, from authors to reviewers to editors to publishing personnel, to obtain controlled access to one common database. The user will receive step-by-step instructions on how to use the software.

 

The Rapid Review system will provide AQCH's participants with numerous benefits, among them:

Authors may submit their manuscripts online. The site will ask for contact information and will then allow the author to upload files and images in multiple formats. The uploaded files will be compiled into a PDF version of the manuscript, available for review.

Authors and reviewers can login wherever they have access to the Internet and update their contact information online, thus facilitating rapid communication. Reviewers will be able to read assigned manuscripts and critique the article and images online. Reviewer expertise can be searched to match keywords that the author provides. This will facilitate matching of papers and reviewers. Authors can login and view the status of their paper at any time during the process.

Authors and reviewers will be notified of review decisions electronically once the editorial offices posts the decision.

Rapid Review is a huge step toward digitizing all aspects of the manuscript process. It provides a faster, more efficient, seamless link from submission through the editorial and peer review process, right through to publication.

For manuscripts that have been accepted for publication in the journal, download and submit the following forms: Copyright and Financial Disclosure Forms

This completed checklist and a DISKETTE/CD-ROM must accompany each manuscript submitted to AQCH. If incomplete, the manuscript will be returned to the author for correction before review.

     Provide a statement verifying that (1) the manuscript is original, (2) all the authors were active participants,
(3) you wish to submit the manuscript for publication in AQCH, and (4) the manuscript is not currently under consideration for publication in another journal.
    Provide complete financial disclosure information.
 
General
      Submit 3 sets of the double-spaced manuscript (reference list also double spaced), with ample margins. 
      Limit the length of the text to approximately 3,000 words. 
      Number the pages consecutively, starting with the title page, and continuing in the following order: abstract and keywords, body of text, references, figure legends and tables.
      Provide signed permission forms from the copyright holder and original author for all previously published tables or figures.

    Cite all references, figures (in arabic numerals) and tables (in roman numerals) in the text.
    Use generic names of drugs when possible. For any trade name cited, capitalize the first letter, and include the manufacturer's name and location (city, state/province, country).  
    Express weights and measures in metric values.
 
Title Page
     Provide the author's academic degrees (e.g., M.D.) and positions (e.g., Professor) and the corresponding author's name and mailing address, telephone and fax numbers, and E-mail address. 
     Limit the number of authors to 6. If more than 6 authors contributed significantly to the article, the editorial office will consider including their names upon request.
     Acknowledge any presentation at a national or international medical society and any substantive financial support.
   
Abstract Page 
     Submit a structured abstract limited to 200 words: Objective, Study Design, Results and Conclusion. For case reports the headings should be: Background, Case and Conclusion. 
     Address the study power to detect a difference if the research identified important variables that lacked a statistically significant difference.
   
Materials and Methods
      State that the original research was approved by the local institutional review board. 
      Clearly state the method of any randomization, blinding and selection of specific statistical tests.
      Explain dropouts/exclusions and confounding variables.
   
References
      Cite journal articles as follows: Authors: Article title. Abbreviated journal name. Year;volume: inclusive pages. Use Index Medicus abbreviations of journal names. 
       Cite books as follows: Authors: Name of chapter. In Name of book. Editors' names. Place, Publisher, year, inclusive page numbers.
       Identify references in the text with superscript numerals: e.g., Dorin et al.5 
       Number all references consecutively as they appear in the text. 
       Avoid listing unpublished data or manuscripts, personal communications, web sites and non Cpeer-reviewed publications.
   
Figures
     Submit 3 prints for each figure. (Color illustrations in excess of 2 figures [1 image each] will be published at the author's expense.)
     All photographs (line art, grayscale and color) must be in the form of glossy prints surrounded by a 0.5-in, white border. (Slides are not acceptable.)
     Ensure that each photograph or section (i.e., A, B, etc.) is at least 3.5 inches wide. 
     Scan images at the highest dpi possible. Do not modify a low-resolution image by increasing the dpi. Send files at no less than 300 dpi in one of the following formats: TIFF, GIF, JPEG, BMP, PHOTOSHOP, PICT or EPS. Send color files in CMYK mode for 4-color separation.
     Provide names of stains and magnification, and clearly identify critical areas of radiographs, photomicrographs, etc.
     Do not write on either the front or back of figures. Do not attach figures to any type of paper. Use self-sticking notes to indicate numbers, directional position and corresponding article title. Do not place legends on the figure itself; place them all on 1 page.
   Avoid using paper clips or tape on figures.
   
Tables 
     Number and title all tables.
     Provide sufficient information for the table to stand alone.
     Avoid tables in which the data could be included in the text in several sentences.
