Instructions to Authors
These instructions for authors, along with information about copyright, can also be found on the Internet: access under http://www.elsevier.com/locate/knee.

Where to send the manuscript
Three manuscripts (the original and two complete copies including illustrations) should initially be submitted to one of the Editors-in-Chief: John H. Newman, 2 Clifton Park, Bristol BS8 3BS, UK; Tel: +44 (0) 117 906 4213; Fax: +44 (0) 117 973 0887; e-mail newman2cp@aol.com or Richard S. Laskin, Cornell University Medical College and the Hospital for Special Surgery, 535 East 70th Street, New York, NY 10021, USA; Tel: +1 (212) 606-1041; Fax: +1 (212) 249-4653; e-mail laskinr@aol.com

The author should retain a copy for his or her own use. The Journal will not be responsible for manuscripts lost in the mail and the choice of Editor to which an author sends a manuscript, is left to the authors' discretion.

Please do not send a computer disk containing the manuscript with the initial submission.

Preparation of the Manuscript
1. All publications will be in English. Authors whose 'first' language is not English should arrange for their manuscripts to be written in idiomatic English before submission
2. A separate title page should include the title, authors' names and affiliations, and a complete address for the corresponding author including telephone, fax numbers and e-mail address.
3. Authors should supply up to five keywords which may be modified or added to by the Editors.
4. An abstract not exceeding one paragraph of 250 words should appear at the beginning of each Article.
5. Acknowledgements should be included after the end of the Discussion and just prior to the References. Include external sources of support.
6. The text should be ready for setting in type and should be carefully checked for errors. Scripts should be typed double-spaced on one side of the paper only. Please do not underline anything, leave wide margins and number every sheet.
7. All illustrations should accompany the typescript, but not be inserted in the text. Refer to photographs, charts, and diagrams as 'figures' and number consecutively in order of appearance in the text. Substantive captions for each figure explaining the major point or points should be typed on a separate sheet.
8. Tables should be presented on separate sheets of paper and labelled consecutively but the captions should accompany the table.

Summary of Overall Arrangement of Manuscripts
You should arrange your contribution in the following order:
1. Title page including the article title, author(s), affiliation(s), keywords and one author identified for correspondence.
2. An abstract outlining the purpose, scope and conclusions of the paper.
3. The text suitably divided under headings. (Frequently Introduction, Material or Patients, Methods, Results, Discussion will prove satisfactory)
4. Acknowledgements (if any).
5. References.
6. Tables with captions (each on a separate sheet).
7. Captions to illustrations (grouped on a separate sheet or sheets).
8. Illustrations, each on a separate sheet containing no text.

Illustrations
Authors must submit one original and two copy sets of all illustrations with the three sets of manuscripts. When the article is accepted, illustrations should also be submitted in an electronic format wherever possible. All illustrations should be identified with the author's name and figure number marked in pencil.

Photographs
Supply glossy black and white un-mounted prints, plus two photocopies are required. A scale, where appropriate, should be marked on the photograph or included in the caption. Colour reproduction is available if the author is willing to bear the additional printing costs.

For full instructions on the electronic submission of artwork, please visit the following web address: http://www.elsevier.nl/locate/authorartwork.

References
Indicate references to the literature in the text by superior Arabic numerals which run consecutively through the paper. Where you cite a reference more than once in the text, use the same number each time. References should take the following form:
1. Amis AA, Dawkins GPC Functional anatomy of the anterior cruciate ligament. J Bone Joint Surg [Br] 1991; 73B: 260-267
2. Insall JN. Surgery of the Knee. New York: Churchill Livingstone, 1984
3. Graf B. Isometric placement of substitutes for the anterior cruciate ligament. In: Jackson DW, Drez D, eds. The anterior cruciate deficient knee. St Louis: CV Mosby Co, 1987: 102-13.

Please ensure that references are complete, i.e. that they include, where relevant, author's name, article or book title, volume and issue number, publisher, year and page reference.

What information to include with the manuscript
1. Having read the criteria for submissions, authors should specify in their letter of transmittal whether they are submitting their work as an Original Article, Review Article, Short Communication, Case Report, or Meeting Report.
2. A letter of transmittal should be included stating that each author has been involved in the design of the study, interpretation of the data, and writing of the manuscript. Include a statement, "Each of the authors has read and concurs with the content in the final manuscript."
3. Authors are encouraged to suggest referees although the choice is left to the Editors. If you do, please supply the postal address and the email address, if known to you.
4. Only papers not previously published will be accepted; each manuscript must be accompanied by a statement signed by all co-authors that the material within has not been and will not be submitted for publication elsewhere except as an abstract. Emphasis will be placed upon originality of concept and execution.
5. Papers involving human experiments should contain a statement of ethical approval for work undertaken with human subjects.

Preparation of Supplementary Data
Elsevier now accepts electronic supplementary material to support and enhance your scientific research. Supplementary files offer the author additional possibilities to publish supporting applications, movies, animation sequences, high-resolution images, background datasets, sound clips and more. Supplementary files supplied will be published online alongside the electronic version of your article in Elsevier web products, including ScienceDirect: http://www.sciencedirect.com. In order to ensure that your submitted material is directly usable, please ensure that data is provided in one of our recommended file formats. Authors should submit the material in electronic format together with the article and supply a concise and descriptive caption for each file. For more detailed instructions please visit our Author Gateway at http://authors.elsevier.com.

Review and Publication Process
1. You will receive an acknowledgement of receipt of the manuscript by the Editorial Office before the manuscript is sent to referees. Please contact the appropriate Editor if you do not receive an acknowledgement.

Following assessment one of the following will happen:
A: The paper will be accepted directly. In this instance a single copy and illustrations will be kept by the Editorial Office. Two copies will be returned to the corresponding author who will be requested to send the final completed manuscript plus illustrations and the manuscript cover sheet to the Editor. Authors must also submit a computer disk (5.25" or 3.5" HD/DD disk, Zip-disk or CD-ROM) containing the final version of the manuscript. Please refer to the following web address for full instructions on electronic submission http://www.elsevier.nl/locate/disksub. The corresponding author will be notified of acceptance by email or letter. The accepted manuscript and disk will be sent by the Editor to Elsevier, for publication. The disk should be clearly labelled with the title of the paper and authors. The name of the computer and word processing package used should be on the disks and the name of the specific file indicated. A copy of the file and illustrations should be kept by the author.

B: The paper will be accepted subject to minor amendments. Again two copies and illustrations will be returned for correction and one copy will be kept by the Editor. The corrections should be made and the two corrected copies and illustrations sent back to the Editor for checking. Once the paper is accepted the procedure as in 'A' above will be followed.

C: The paper will be rejected but resubmission invited after a major revision. Two copies will be returned along with comments from the Editor or Referees. A complete resubmission of three copies with illustrations is then required as the paper will be re-evaluated by referees and the assessment process will start again.

D: The paper will be rejected outright as being unsuitable for publication in The Knee. One copy will be kept by the Editor, the remaining material will be returned to the author.
2. By submitting a manuscript, the authors agree that the copyright for their article is transferred to the publisher if and when the article is accepted for publication. http://www.elsevier.com/homepage/authors/?main=/homepage/about/ita/copyright.shtml.
3. Page proofs will be sent to the corresponding author for correction, although at this stage any changes should be restricted to typographical errors. Other than these, any substantial alterations may be charged to the authors. Proofs will be sent preferably by e-mail as a PDF file (although they can be sent by overland post) and must be rapidly checked and returned.
4. An order form for reprints will accompany the proofs.

Offprints and Reprints
The principle author will receive 25 offprints of the paper free of charge upon publication. Extra offprint copies (minimum 100) can be ordered prior to publication. Reprints (minimum 100) can be ordered at any time from Elsevier, The Boulevard, Langford Lane, Kidlington, Oxford OX5 1GB, UK.

Publication condition
A manuscript submitted to this journal can only be published if it (or a similar version) has not been published and will not be simultaneously submitted or published elsewhere. A violation of this condition is considered fraud, and will be addressed by appropriate sanctions. Two manuscripts are considered similar if they concern the same hypothesis, question or goal, using the same methods and/or essentially similar data.
