About the journal
The Journal of Hand Surgery publishes original, peer-reviewed articles related to the diseases and conditions of the upper extremity. Both clinical and basic science manuscripts are considered. The journal also publishes Case Reports, which enhance the understanding of the diagnosis, treatment and pathophysiology of conditions of the upper extremity. Special features include Department of Technique articles, which emphasize the technical aspects of surgery, and Clinical Perspective reports, which place current controversial topics in their proper perspective.
Journal of Hand Surgery is ranked 20th out of 42 Orthopedics titles on the 2002 ISI Journal Citation Reports.
Instructions to Authors
General Information
The Journal of Hand Surgery welcomes original articles in English relating to any aspect of surgery of the hand and upper extremity. Authors lacking facility with English syntax should seek the appropriate editorial assistance prior to submitting their manuscript. Material requiring major editorial work will be returned without review. Authors must submit three complete printed copies of the manuscript, prepared as specified below. The Journal utilizes anonymous peer-review in evaluating manuscripts for publication. In accordance with Copyright Act of 1976, all manuscripts must be accompanied by a Copyright Assignment form signed by the corresponding author. It is assumed that the corresponding author speaks for his or her co-authors and certifies that all listed authors participated meaningfully in the study and that they have seen and approved the final manuscript. When submitting a paper the author(s) must make a full statement to the Editor about all submissions and previous reports that might be regarded as prior or duplicate publication of the same or very similar work. Copies of such material should be included with the submitted paper to help the Editor decide how to deal with the matter.
The letter should describe any commercial affiliation or consultancy of an author that could be construed as a conflict of interest with respect to the submitted data. If the Editor finds that such activities raise the appearance of a conflict, he may recommend disclosure in a footnote. Otherwise, the information will remain confidential.
Ethics: Do not use patients' names, initials, or hospital numbers. Articles emanating from a particular institution are assumed to be submitted with the approval of the requisite authority.
Human subjects: Articles involving research conducted in human subjects must include a statement in the Materials and Methods section indicating approval by the institutional review board and noting that informed consent was obtained from each patient. For reports of research using human subjects, provide assurance that (a) informed consent was obtained from each patient and (b) the study protocol conformed to the ethical guidelines of the 1975 Declaration of Helsinki as reflected in a prior approval by the appropriate institutional review committee. individual patients should be referred to by number, not by initials.
Animal experimentation: Manuscripts reporting animal experiments must include a statement in the Materials and Methods section that animal care complied with the guidelines of the authors' institution and the National Institutes of Health and any national law on the care and use of laboratory animals.
Financial interest: Authors must disclose any commercial affiliations that might create a conflict of interest with respect to an accepted manuscript. A conflict-of-interest statement should be submitted along with the manuscript.
Permissions: Lengthy direct quotations, tables, or illustrations taken from copyrighted material must be accompanied by written permission for their use from the original copyright holder. Permission request forms are available from the Publisher (215/238-6446). The permission is presented as a text or table footnote or as an addition to the figure legend and must provide complete information as to source. Photographs of identifiable persons must be accompanied by a signed release that indicates informed consent.
Units of measurement: Use Systeme International (SI) measurements.
Statistics: Describe statistical methods with enough detail to enable a knowledgeable reader with access to the original data to verify the reported results. When possible, quantify findings and present them with appropriate indicators of measurement error or uncertainty (such as confidence intervals). Avoid sole reliance on statistical hypothesis testing, such as the use of p values, which fails to convey important quantitative information.
Abbreviations: If it is used more than three times in the abstract and more than four times in the article, spell out an abbreviation the first time it is used, followed by the abbreviation in parentheses. The abbreviation may be used subsequently. Spell out all abbreviations at the beginning of a sentence.
Drug names: Use generic names. The proprietary name may be mentioned in parentheses, along with the name and location of the manufacturer.
Revised manuscripts: Must be submitted in triplicate and accompanied by a cover letter that addresses all the reviewers' observations. This letter should detail point by point which changes have been made and should give reasons for those recommended changes that have not been made. The manuscript number must appear in the cover letter.
Organization of the manuscript: Failure to comply with these requirements will delay consideration of the manuscript. The manuscript may be returned to the author(s) for appropriate modifications in format prior to review. Since the authors' identities are withheld from the reviewers, authors' names are to appear only on the title page. Submit 3 complete sets of the manuscript, typed single-sided and double-spaced throughout (including the list of references, tables and figure legends) on 22 x 28 cm (8?x 11 inch) white paper with 2.5 cm (1 inch) margins all around. Arrange the manuscript as follows: title page, abstract, introductory paragraphs, materials and methods, results, discussion, references, figure legends, tables, figures. Number all pages consecutively beginning with the title page.
Title page: The title should be a concise and informative description of the study. Include a short title, maximum length 45 characters, to be used as the running head; the running head should not include identification of the author(s). Also provide up to 5 key words for indexing and retrieval.
Include the authors' highest academic degrees, along with the department and institution where the work was done. Designate one author as the correspondent and supply his or her complete mailing address, telephone number, fax number, and e-mail address. If the name or address for reprint requests is different, so state. Every person listed as an author should have materially participated in the design, execution, and analysis of the study and should verify the accuracy of the entire manuscript before its submission. Lesser contributions may be noted in an acknowledgment section at the end of the manuscript. Manuscripts should have no more than 6 authors; a greater number requires justification.
Include disclaimers, if any, and a brief acknowledgment of grants or other assistance.
Abstract: Include a 1-paragraph abstract of 150 words or less that concisely states the purpose of the study, the measurement parameters utilized, the objective findings, and conclusions of the work performed. Do not include footnotes or references in the abstract. Type the abstract on a separate page.
Text: The text of the articles is usually, but not necessarily, divided into sections with the headings Introduction, Materials and Methods, Results, and Discussion. Long articles may need section subheadings to clarify their content. Do not repeat in the text all data in the tables or illustractions; emphasize or summarize only important observations. A conclusion may be included only if it is not redundant with the abstract or with a summarization paragraph of the discussion. Avoid claiming priority.
References: Authors are responsible for verifying the accuracy and completeness of references. References should not be merely a listing of the results of a computerized literature search, but should have been read by the author and deemed pertinent to the manuscript. Type references double-spaced on sheets separate from the text and numbered consecutively in chronologic order of citation in the text. Identify references in the text by superscript arabic numerals. List all authors when 7 or fewer; when more, list the first 6 and add et al. Use abbreviations of journal titles conforming to Index Medicus. Include complete opening and closing page numbers for each citation. With the submitted manuscript, attach a copy of the first page of each article or the front page of each book cited so that the accuracy of the reference can be checked. A Medline print-out is also acceptable for journal references.
Avoid using abstracts as references. Do not cite personal communications and unpublished material (including oral and poster presentations, correspondence club letters, and manuscripts not yet accepted for publication) in the reference list. If critical to the manuscript's message, written personal communications and other unpublished material may be included in the text and referenced in parentheses. Please note the following examples of reference style.

Tables: Double-space table data with the table number and title centered above the table and with explanatory notes below the table. Each table should be on a separate page. Do not duplicate material in tables with material in the text or figures.
Figure legends: Number the figures with Arabic numerals in the order mentioned in the text. Provide sufficient explanation to render the figure intelligible without reference to the text. Define all symbols and all abbreviations not yet spelled out in the text. For any copyrighted material, indicate that permission has been obtained (see Permissions, above). Figure legends should be typed consecutively on a separate sheet of paper.
Figures: Submit 3 complete sets of illustrations (glossy black-and-white prints at least 10 x 15 cm) unmounted and in envelopes. Use professionally produced arrows to highlight the photograph's message. Do not write on the illustrations. On adhesive-backed labels, type the figure number and place an arrow to indicate the top of the photograph. Do not write the authors' names or names of institutions anywhere on the figures. The figure title and caption material appear in the legend, not on the figure. Double-space legends (maximum length, 40 words) sequentially on sheets separate from the text. Do not use a separate page for each legend. Figure numbers correspond with the order in which figures are presented in the text. Do not send original artwork or x-ray films. Do not include photographs or x-ray films of normal findings. Line art should be submitted with no gradations of shading, as they will not reproduce well. Use cross-hatching or patterns where shading is necessary. Special arrangements must be made with the Editor for color illlustrations, and the cost must be borne by the author(s). Approximate cost is $1,500 for the first page (whole or partial). The Publisher will provide a cost estimate once a manuscript has been accepted. If photographs of persons are used, either the subjects must not be identifiable or the person's written permission to use the photograph must accompany the manuscript. If a figure has been published, acknowledge the original source and submit written permission from the original copyright holder to reproduce the material (see Permissions, above).
SPECIAL FEATURES:
Department of Technique articles will be considered for publication if they make important contributions to the technical aspects of hand surgery. The manuscripts should include a brief Abstract, Introduction, Technique, and Discussion.
Case Reports will be considered for publication if they contribute to an understanding of the diagnosis, treatment, and/or pathogenesis of diseases or conditions related to hand surgery. The manuscripts should include a brief Abstract, Introduction, Case Report, and Discussion.
Books will be reviewed depending on their interest and value to the reader. Two copies of the book to be reviewed should be sent to the Editor. No books will be returned.
Letters to the Editor of sufficient interest will be considered for publication. Letters should be typed double-spaced and limited to 500 words. Letters related to previously published articles must be submitted within 3 months of the article's publication; a copy of the letter will be sent to the author(s) for an invited response. Letters are subject to correct editorial policies.

Reprints: A reprint order form is sent with page proofs to the author. Reprints should be ordered prior to publication because those ordered after publication are significantly more expensive. Questions regarding reprints should be directed to the Publisher (215)238-5534).
Submission of manuscripts on disk: Manuscripts must be submitted on a disk, preferably in Microsoft Word. A double-spaced hard copy version of the final manuscript, free of hand-written alterations, must accompany the disk. All components of the manuscript must appear within a single electronic file: references, figure legends, and tables must appear at the end of the manuscript. Please refrain from using end notes as references or automatic list numbering because these features are lost in conversion: simply type the reference number in parentheses in the text and type the reference list. Formatting, such as Greek letters, italics, super- and subscripts, may be used: the coding scheme for such elements must be consistent throughout.
Electronic illustration submission: Figures may be submitted in electronic format. Images should be provided in EPS or TIF format on Zip disk, CD, floppy, Jaz, or 3.5 MO. Graphics software such as Photoshop and Illustrator, not presentation software such as PowerPoint, CorelDraw, or Harvard Graphics, should be used to create the art. Color images must be CMYK, at least 300 DPI, with a digital color proof, not a color laser print or color photocopy (this proof will be used at press for color reproduction). Gray scale images should be at least 300 DPI and accompanied by a proof. Combinations of gray scale and line art should be at least 1200 DPI and accompanied by a proof. Line art (black and white or color) should be at least 1200 DPI and accompanied by a proof. Please include hardware and software information, in addition to the file names.
Authors are responsible for applying for permission for both print and electronic rights for all borrowed materials and are responsible for paying any fees related to the applications of these permissions.
Copyright assignment form authors' release

General Requirements:
- The disk copy must match exactly the hard copy of the manuscript except for Figures, which are submitted separately. Use 3?" diskettes only. For MacIntosh software, the diskette must be high density.
- Most word-processing formats are acceptable (Microsoft Word, WordPerfect); Microsoft Word is preferred. If the program used is nonstandard, the material also should be provided in text format on a separate disk, marked as "text file."
- Include the manuscript title and author name(s) on the diskette label.
File Organization and Formatting:
- All components of the manuscript must appear within a single electronic file, on a single disk (do not include multiple files or backup versions of the file).
- Typographical formatting (column widths, type styles, etc) will be handled by the publisher. Do not use this type of formatting.
- Editorial formatting (use of italics, superscripts, Greek letters, etc) may be included in the disk file.
The coding scheme for such elements must be consistent throughout the file.
Text Style:
- Type text flush left (do not indent paragraphs). Use two carriage returns at the end of a paragraph to separate it from the one that follows.
- Enter only one space between words and after sentences. Use 1 carriage return ("hard return") only after a heading and use 2 carriage returns at the end of a paragraph. Allow line breaks within a paragraph to be determined automatically by the word-processing program. If it is necessary to force the end of a line within a paragraph, use the program's "soft return" feature.
- Do not use the program's indenting or margin-setting features. (These will be added during typesetting.)
NOTE: The diskette version of the manuscript must be identical to the submitted hard copy (free of handwritten editorial alterations).
