Instructions to Authors

PREPARATION OF TYPESCRIPTS

Material submitted to the IJCP should be written with a UK and overseas general medical audience in mind, carefully prepared for publication and pruned of unnecessary words and phrases. Headings within the text should be used to highlight the context.

All materials should comply with the uniform requirements for manuscripts submitted to biomedical journals, prepared by the International Committee of Medical Journal Editors(see BMJ 1988; 296: 401-05).

Two copies of the typescript, plus disk are required; authors should retain a further copy for reference. All sections should be typewritten in double spacing on one side of the paper with margins of not less than 3cm. All pages, including tables and figures, should be numbered.

Typescripts (and illustrations) not accepted for publication will be returned only if a stamped addressed envelope is provided.

Submission of diskettes
Articles must be saved in Word format and submitted on IBM-compatible 3.5" floppy disks. Submissions via email are not acceptable. Disks should be posted together with a hard copy to the address below. All disks should be suitably packaged to avoid physical damage or data corruption. If an author has to revise an article, then a new disk must be submitted.

Title page
This should contain (1) a concise, informative title; (2) the names and initials of all authors, their qualifications (maximum two per author) and their appointments; (3) the department(s) and institution(s) where the work was carried out; (4) the name and address of the author responsible for correspondence concerning the typescript, proofs and published article.

Summary
The second page should contain a summary of no more than 150 words. The summary should not contain any subheadings.

Tables and illustrations
Only those essential to the paper should be included. They should be separated from the text, be numbered in the order in which they are mentioned in the text, and each should have a short explanatory caption. Any tables or illustrations previously published should be accompanied by the written consent of the copyright holder, an acknowledgement should be included in the caption, and the full reference included in the list. Tables should be typed in double spacing on a separate sheet. Each table column should have a short heading.

Illustrations should be provided in camera-ready form, with redundant areas trimmed; the figure number, the author's name and the orientation (the word "top"and an arrow) should be marked on the back. They should be professionally drawn, labelled and photographed; freehand lettering is not acceptable. Lettering/numbering should be sufficiently large to ensure legibility after reduction for publication. Line drawings may be submitted as photographic prints or high-quality photocopies. All other illustrations should be black and white photographic prints (usually 127 x 173mm). Staining techniques and the magnification of micrographs should be stated, and arrows/abbreviations explained in the captions. If the subjects of photographs are identifiable, either their eyes should be masked or their written permission to use the photograph be submitted with the typescript.

Illustrations may be reproduced in colour if essential to the article. Illustrations will be returned after publication only if a stamped, addressed envelope is provided.

Measurements and abbreviations
Measurements should be given in the units in which they were made, but non-metric units must be accompanied by metric (SI) equivalents (exceptions: blood pressure in mmHg; haemoglobin in g/dl). British Pharmacopoeia drug names should be used (proprietary/other names may follow in parentheses). If an acronym is used, the term for which it stands should be given in full at its first mention in the text: eg, anti-diuretic hormone (ADH).

Acknowledgements
Only the help of those who have made substantial contributions to the study and/or preparation of the paper should be acknowledged. Source(s) of grant support, equipment and drugs should be included.

References
Authors should verify references against the original documents. The
Vancouver style is used, in which references are identified in the text by superior arabic numerals (unbracketed and after the punctuation mark, eg ;6? ) and are numbered and listed consecutively at the end of the paper in the order in which they are first cited in the text. The full list of references at the end of the paper should include: names and initials of all authors (unless more than four, in which case only the first three are given followed by et al); the title of the paper; the journal title abbreviated according to the style found in the Index Medicus; the year of publication; the volume number (and issue/supplement number if appropriate) and the first and last page numbers. References to books should give the book title, and volume or edition number, if applicable; the publisher and place of publication, the year of publication and relevant page numbers; those of multiple authorship should also include the chapter or article title, the first and last page numbers, and the name and initials of editors. For example:

1. Hennemann G, Doctor R, Krenner EP et al. Raised total thyroxine index but normal free thyroxine. Lancet 1979; i: 639-42.

2. Smyth GDL. Chronic Ear Disease, 2nd edn. Edinburgh: Churchill Livingstone, 1980.

3. Kohner EM, McLeod D, Marshall J. Diabetic eye disease. In: Keen H, Jarret H, Eds. Complications of Diabetes, 2nd edn. London: Arnold, 1982; 57-8.

Papers accepted but not yet published should be included in the references and should be followed by "In press" Those in preparation (including any submitted for publication), personal communications and unpublished observations should be referred to as such in the text only.

Investigations involving humans, the consent of patients and approval of the protocol by an ethical committee should be confirmed. Original textual matter quoted from other authors must have formal citation and be appropriately referenced. Any statements that might be construed as defamatory must be avoided.

Authors are required to assign copyright in their paper to Blackwell Publishing Ltd, and a Copyright Assignment Form must be signed.  Copyright assignment is a condition of publication and papers will not be passed to the publisher for production unless copyright has been assigned.  (Papers subject to government or Crown copyright are exempt from this requirement).  To assist authors the editorial office will supply an appropriate copyright assignment form.

PROOFS

Proofs are sent to the author designated to receive them. They should be returned promptly, and revisions should be kept to a minimum.

REPRINTS

Twenty-five offprints of each article will be issued just after publication to the author designated to receive correspondence.

COLOUR PRINTING AND CASE REPORT

The publishers are prepared to print colour illustrations; the cost to the author is up to ?200 for an eight-page, four-colour article. Processing of case reports is free of charge only if emanating from an IJCP subscriber establishment or individual.
