    NEW SUBMISSION REQUIREMENTS!

Manuscripts must be submitted online. Once you have prepared your manuscript according to the instructions below please visit the online submission web site. Instructions on submitting your manuscript online can be viewed below.

All papers submitted will be evaluated on the basis of scientific merit and for their contribution to an increased understanding of the quality of health care.

The language of the Journal is English as generally used in English-speaking countries. The Journal cannot provide editing services to correct papers to conform to such usage. Accordingly, authors for whom English is not the first language are strongly advised to obtain English-language editing services before submitting papers to the Journal, since reviewers are likely to judge unfavourably papers that cannot be clearly understood.

Manuscripts should include a title page with the name, address, telephone and fax numbers, and e-mail address of the author to whom correspondence should be sent.

    Authorship of manuscripts
All authors must fulfill the criteria of authorship (see Authorship criteria and requirements). The Journal does not impose a limit on the number of authors but may seek further justification of authorship in cases that appear exceptional. Authors may add a publishable footnote explaining order of authorship.

    Group authorship
The Journal encourages authors of multi-site studies to consider group authorship. If authorship is attributed to a group (either solely or in addition to one or more individual authors), all members of the group must meet the full criteria and requirements for authorship described in the following paragraphs. One or more authors may take responsibility 'for' a group, in which case the other group members are not authors, but may be listed, with their permission, in an acknowledgement.

    Authorship criteria and requirements
Necessary criteria for authorship include one or more of the following: substantial participation in the conception and design of the work, execution of the work, analysis of the data, contribution of statistical expertise. All authors should also contribute to the writing of the manuscript and approve the final version. Additional responsibilities are set out in the Authorship responsibility statement below. Contributors to the manuscript who do not meet these criteria may be listed, with their permission, in the Acknowledgements section (see Acknowledgements).

    Authorship responsibility statement
'I certify that I have participated sufficiently in the conception and design of this work and analysis of the data (when applicable) as well as in the writing of the manuscript to take public responsibility for it. I believe the manuscript represents valid work. I have reviewed the final version of the submitted manuscript and approve it for publication bearing my name. Neither this manuscript nor one with substantially similar content under my authorship has been published or is being considered for publication elsewhere, except as described in an attachment (when applicable). If requested, I shall produce the data upon which the manuscript is based for examination by the editors or their assignees.'

    Financial disclosure
Authors who have financial arrangements with for-profit organizations whose services or products, or whose competitors?services or products, are a subject of discussion or evaluation must disclose this relationship when submitting the manuscript. This information will be disclosed only if the paper is accepted for publication and will generally be communicated in the form of an acknowledgement at the end of the text.

    Acknowledgements
Contributors to the manuscript who do not meet the criteria for authorship as set out above (see Authorship criteria and requirements) may be named, with their permission, in the Acknowledgements.

Authors are responsible for obtaining written permission from all persons named in the Acknowledgements, since readers may infer their endorsement of data and conclusions. The corresponding author must include the following statement in the cover letter: 'I have obtained written permission from all persons named in the Acknowledgement.' The signature of the corresponding author under the Acknowledgement(s) statement at the foot of the Manuscript Submission form will fulfill this submission requirement.

Acknowledgements, both those of grant support and those of individuals who were of direct help in the preparation of the study, should be cited in an 'Acknowledgements section at the end of the text before the 'References' section.

    Reproduced material
Authors wishing to reproduce material that has been previously published elsewhere must obtain permission in writing from the copyright holder for the source document.

    Copyright
It is a condition of publication in the Journal that authors assign an exclusive licence to the International Society for Quality in Health Care and Oxford University Press. This ensures that requests from third parties to reproduce articles are handled efficiently and consistently and will also allow the article to be as widely disseminated as possible. In assigning copyright, authors may use their own material in other publications provided that the Journal is acknowledged as the original place of publication, and provided that Oxford University Press is notified in writing and in advance. Address letters of notification to: Rights and New Business Development, Journals Division, Oxford University Press, Great Clarendon Street, Oxford OX2 6DP, UK. Tel: +44 (0) 44 1865 354490 or 353695; Fax: +44 (0) 1865 353485. E-mail: journals.permissions@oupjournals.org; website: www3.oup.co.uk/jnls/permissions

    Changes of address
Please notify both the publishers and the Editorial Office of any change of address occurring during the review or production processes. The address for the Editorial Office is given above (see Editorial Board on the Home page); the publishers?address is: Hilary Lamb, Journals Production Department, Oxford University Press, Great Clarendon Street, Oxford OX2 6DP, UK. Tel: +44 (0)1865 354957; Fax: +44 (0)1865 353798.

    Manuscript preparation
Appropriate length for the text of the paper is 3000-4000 words (12-16 double-spaced typed pages). Use double-spacing throughout the manuscript, including the reference section. The authors?names should not appear within the body of the manuscript or on the figures so that author anonymity may be maintained during the review process where this is considered necessary. Organize the manuscript in the order indicated below, begin each component on a separate page, and include a page number in the upper right corner of each page.

    Title page
The title page of the manuscript should include:
    • title of the paper or article (80 spaces maximum)
    • authors' names
    • authors' affiliations [the name of the department (if any), institution, city, state and country where the work was done] using superscript numerals to link each author with the appropriate institution(s)
    • name, address, telephone and fax numbers, and e-mail address of the author to whom proofs and reprint requests are to be sent
    • running title (not more than 30 spaces)
    • two word counts: one for the abstract and one for the text of the manuscript

    Abstract
Page 2 of the manuscript should include the title of the article followed by the abstract. This should consist of no more than 300 words. With few exceptions, abbreviations should be avoided in the abstract. For original research studies, authors should use the structured format for the abstract1 as outlined in Appendix 1.

Variations on the structure presented in Appendix 1, such as Objective, Design, Main measures, Results, Conclusion, if more suited to the study in question, will be considered.

    Review articles (usually by invitation)
    • Purpose: give the primary objective of the review article
    • Data sources: provide a concise summary of data sources
    • Study selection: list the studies selected for review and cite the reasons for their selection
    • Data extraction: describe the rules developed for abstracting data and how they were applied
    • Results of data synthesis: outline the methods of data synthesis and give the key results
    • Conclusion: record the main conclusions, citing potential applications and research needs.

    Keywords
Up to eight key words or short phrases identifying the topics addressed in the paper should be included at the bottom of the abstract page. These will help in the indexing of the Journal and may be published with the abstract. Where possible, use terms from the Medical Subject Headings list in Index Medicus.

    Text
(See also Appendix 2: Elements of Study Design and Statistical Presentation)

The organization of papers should follow the format described above for structured abstracts. Authors should make every effort to present the contents and processes of the study as clearly and as concisely as possible and to avoid using jargon. As in the case of the abstract, all non-standard terms should be written out in full the first time they are used, and the appropriate abbreviation included in parentheses. Subsequent uses of the term may be represented by the abbreviation.

Please make sure that all references are cited in numerical order in the text and that all tables and figures cited in the text are numbered according to the order in which they appear. Data appearing in tables or figures should be summarized, not duplicated, in the text. All data cited in the text should be checked carefully against the data in the tables to ensure that they correspond, and all the names cited in the text should be checked carefully against the references to ensure that the spelling is correct. Any ambiguous symbols (e.g. the letter "O" versus the numeral 0   the "I"  versus the numeral 1 should be identified. Acknowledgements of grant support and of individuals who were of direct help in the preparation of the study should be included at the end of the text.

    Linguistic Usage
The forms of usage set out in the Oxford English Dictionary will be followed for all papers, with the exception of those submitted using US-English spelling.

The ‘active voice?is the preferred writing style for the Journal, e.g. "we attempted" not "an attempt was made" Other linguistic preferences are "we used" not "the study used""because" not "since" "although" not "while" "after" not "following" and "before" not "prior to"

    References
Type references using double-spacing and number them consecutively in the order in which they are first mentioned in the text, not alphabetically. Identify references in the text, tables and legends by Arabic numerals. References cited only in tables or figure legends should be numbered in accordance with a sequence established by the first mention in the text of a particular table or figure.

Authors are responsible for the accuracy and completeness of the references. References should be presented in the Vancouver style.2 For journal articles the following information should be included:
    • all author names (surnames followed by initials); if more than six authors, give the first three followed by "et al"
    • title of the article
    • the journal title which may be abbreviated according to the rules set out in Index Medicus
    • the year of publication
    • the volume number
    • the page span

For books, include the surname(s) and initials of the author(s) or editor(s) of the book, title of the book (including volume and edition number, if appropriate), publisher’s name and location, and year of publication. In the case of a journal article, book, or other reference published in a language other than English, translate the title into English and identify the language of the original in parentheses, for instance in Spanish.

Examples of the correct formats are as follows:

    Journal article
    1. Laouri M, Kravitz RL, Bernstein SJ et al. Underuse of coronary angiography: application of a clinical method. Int J Qual Health Care 1997; 9: 15?2.

    6. Federal Ministry of Labour, Health and Social Affairs, Austria: http://www.bmags.gv.at Accessed [Date (i.e. date reference item accessed on organization website)].

‘Unpublished observations?and ‘personal communications?should not appear in the references. These should be inserted in parentheses in the text, and letters of permission from all individuals cited in this way should accompany the manuscript. Information obtained from manuscripts that have been submitted for publication but not yet accepted should be cited in the references: include authors and manuscript title followed by ‘submitted for publication.?Manuscripts that have been accepted for publication but have not yet been published may appear in the references: include the authors, manuscript title, and name of the journal followed by "in press"

    Tables and figures
Print all tables and figures one to a page and attach at the end of the manuscript.

    Tables
The total number of tables and figures should not exceed eight. Number tables in the order in which they are cited in the text. Every column in the table should have an abbreviated heading. Define all abbreviations and indicate the units of measurement for all values. Explain all empty spaces or dashes. Indicate footnotes to the table with superscript Arabic numbers cited in order as you read the table horizontally.

    Figures
Figures must be supplied in electronic form via the submission system, see the online submission instructions for file requirements. Letters, numbers and symbols should be clear throughout and should be large enough to remain legible when reduced for publication. Be sure that all spelling is correct, that there are no broken letters or uneven type, and that abbreviations are consistent with those in the text.

Submit photomicrographs in the final desired size.

    Figure legends
Legends should be typed on a separate page from the figure(s), double-spaced, and numbered with Arabic numerals corresponding to the figures. When symbols, arrows, numbers or letters are used to identify parts of a figure, each should be explained clearly in the legend or as a footnote. The legend should permit the figure to be understood without reference to the text. If the figure has been published previously a credit line should be included and a letter of permission to reproduce signed by the copyright holder of the original published version must accompany the manuscript submission.

    Letters to the Editor
Letters to the Editor are welcome and will be published at the discretion of the Editor, as space allows, and subject to editorial review, editing and abridgement. Preference will be given to letters addressing matters raised by papers published in recent issues of the Journal. Letters should not exceed 500 words and 10 references. Only letters selected for publication will be acknowledged. Copyright licencing will be requested as a condition of publication.

    Production process
Decisions of the Editor are final. All material accepted for publication is subject to copyediting.

Authors will receive page proofs of their articles before publication, and should answer all queries and carefully check all editorial changes at this point. Authors are responsible for the scientific content of their articles. Corresponding authors of all articles (but not Letters to the Editor) can choose to receive either a free URL for the online paper or 25 free offprints. Additional offprints may be ordered directly from the publisher. An order form will be sent with the page proofs.
