Authors must accept full responsibility for the published paper and must meet all the following criteria: (1) have conceived or designed the work resulting in the submitted paper or interpreted the evidence arising from that work; (2) have written the paper or reviewed and revised it; and (3) have approved the final version. Additional criteria are spelled out in "Uniform requirements for manuscripts submitted to biomedical journals."

A cover letter signed by all the authors should state that the manuscript has not been published and is not under consideration by any other journal. It should explain the purpose of the paper and its intended readership. Authors must disclose the source of any financial or material support, any commercial interest they may have in the subject of the study and any involvement with an organization that has a financial interest in the research materials or the topic. Also included, if appropriate, should be a signed letter of permission from any patient whose case is described in the paper, people identified as sources of personal communications, those acknowledged or identifiable in illustrations, and from the copyright holder of previously published material that is to be reproduced, with or without modification.
